Boarding Form

***Boarding charges are applied on the date of drop off. There is no charge for the pickup date if a pet is collected by 11:00am.

Drop off and pick up times of pets when hospital is closed must be prearranged.

If your plans change, please contact us to make a revised boarding agreement. Pets that have not been claimed within seven days of boarding agreement will be considered abandoned and become the property of Abbott Animal Hospital. Abbott Animal Hospital then reserves the right to care, custody and control of the pet.

Science Diet dry food is offered to boarding pets twice daily. Dogs are exercised outside on our nature trails frequently throughout the day. Cats are handled and pet several times daily. There will be an additional charge for supplemental food for in appetent or sick pets.

Special feeding instructions for food brought from home:

Boarding pets must have current vaccinations. Dogs require Rabies, Bordetella, DA2PPC. Cats require Rabies and FVCRP. If your pet needs vaccinations or you would like to have your pet examined by a doctor while boarding, please make your request or describe the symptoms your pet has.

All boarding pets must be free of external parasites such as fleas and ticks. If pets are found to have external parasites, they will be treated with Advantage at the owner’s expense.

We do our best to minimize the stress of boarding. Some pets develop diarrhea or vomiting due to diet change or stress of being in a new environment. Pets may have undetected disease manifest.

We suggest leaving only towels and t-shirts with pets for cage comfort. We are not responsible for lost, destroyed or soiled items.

I am the owner/agent of the above listed pet(s) and assume responsibility for all charges incurred for care. I understand and agree to all of the above terms. I understand that full payment is due when my pet is discharged.
